Observation

WAC 110-300-0165(3)(a): The playground has two HVAC units that are accessible to the children and have opeinings large enough to get their fingers and other objects into. One HVAC had a toy sitting on top of it. The kitchen has butcher knives sitting on the counter accessible to the children. WAC 110-300-0165(5)(a): There is a large bin outside on the playground that is cracked across the top and has a sharp edge where it is cracked. WAC 110-300-0165(5)(a): The school age 2 room has some outlets that are not tamper resistant with automatic shutters that only allow electrical plugs to be inserted. WAC 110-300-0165(5)(g): The kitchen outlet just above the kitchen sink had two electrical cords plugged into it with one of the cords that was plugged into the outlet having a live end that was not plugged into anything (phone charger).
