110-300-0165(3)(a) Licensee has three broken deck boards with a protruding nail on it accessible on the front deck. 110-300-0165(3)(d) Licensee has a second freezer on the porch that is accessible and poses an entrapment hazard. 110-300—0165(3)(f) licensee has one white shelf in the hallway, one brown shelves in the hallway, and two black shelves in the kitchen that are not anchored and pose a risk of a tipping hazard. 110-300-0165)3)(g) Licensee has flaking paint on green deck in several places, chipping paint in the bathroom trim and door, and in the living room trim.
