During onsite inspection, Licensor reviewed four enrolled children’s files and observed two children’s CIS immunization records incomplete and missing MMR, DTAP, Polio, Varicella, PCV, and HIB vaccines, and no 30-day written plan to get children updated of Certificate of Exemption was on file.
