Observation

Neither staff outside in ratio during incident saw the incident occur, only heard child crying. Both staff gave different stories of what staff were doing that prevented them seeing child be injured. Several incident's of concern surrounding staff member on cell phone while in ratio and supervising children. That staff disclosed phone usuage during incident. Staff member A.N. was terminated from employment after repeated cell phone usuage while supervising children.
