Observation

Based on the information gathered from the interviews, incident reports and staff's notes, it was determined that the early learning provider failed to report by telephone within 48 hours to the listed individuals, department, and other government agencies when the provider knew and had reason to know of a suspected physical, sexual or or emtional child abuse.
