Observation

Child was left unattended in the outdoor play area for up to 5 minutes due to a lack of proper supervision by classroom support staff. Miscommunication and lack of head counts with face to name system and utilizing the centers electronic accounting program led to this child being left outdoors under a play structure for up to 5 minutes. Treachers each believed that one another had the child with them when neither one actually did have account of this child. Lack of knowledge of supervision plan on the part of teachers contributed to this child being left alone.
